Procedure of Basti karma with clinical experience
Basti
is a specialised therapy in panchakarma the herbal liquid medicine is cleanses
the doshas which are disturbed in the umbilicus, lumbar region, side of the
chest and pelvic region. Basti therapy is specially meant for the vajata
vikaras, neurological disorder. The application of basti helps not only
regulating and co-ording vata dosha in to its site but also controls the other
doshas involved in the pathology of the disease.
Some
acharyas told basti is arda chikaitsa but I would like to clarify that 50% of
the disease can be managed by basti alone. It quick action produces the
dissolved effect like shodana, tarpana, rasayana, vajeekarana effect. So
acharya kashyapa enumerated basti treatment as an amrutam.
Gudam
is damani marma, mamsa marma and sadya pranahara marma. So physician take care
about the anatomical importance of the gudam while inserting the basti netra.
The route word of basti has got different meaning ‘vasu nivase’, ‘vasti na
diyate’ etc. Rational thinking on various aspect of basti karma has brought
about the following classification, Adhistana bedha, Dravya bedha, Sankya
bedha, Matra bedha.
In
Dravya Bheda basti is classified into niruha basti and Anuvasana basti
depending on dravyas used niruha basti is again classified into Madhutailika
basti, Yutarata basti, Sidda basti, Yapana basti, Shodana basti, Uthkleshana
basti, Karshana basti, Rasayana basti, Vajikarma basti. Anuvasana basti is
classified into Sneha basti, Anuvasana basti and Matra basti. According to
Sankya bheda basti is classified into Karma basti, Kala basti and yoga basti.
Matra bheda, Basti is classified into Dwadasha prasuta basti, prasruta yogagika
basti, padahina basti.
Anuvasana
basti is a type of sneha basti. It is mainly for vataja disorder, in stantensly
produces clarity to the mind, promotes energy, strength, completion, agni,
person becomes young & butyfull as a tree irrigated with water at the root
produces green leaves, flowers, fruits. On the day before administration of
anuvasana basti ‘kosta shuddi’ should do with the Anulomana / Virachana dravyas
this process is helps to absorption of basti dravya. Usully all the practionar
using chikkanapa or madyama paka taila are used for the Anuvasana basti. Satahva
kalka and saindava are to be added to the oil of anuvasana basti. In the
praporation of 6gm, 4gm, 2gm for maximum moderate and minimum quantity of
sneha, we observed better therapeutic effect when this combination is added to
the Anuvasana taila. This helps in increasing vata shamana property of oil and
retaining time is also less. This is very helpful and get very good result.
Usually mudga yusha, ksheera, mamsa rasa is given for kapha, pitta, vata
respectively susruta says sneha basti has to give immediately after the meals
even before the hand become dry. In olden days basti putaka & netra is used
for Anuvasana basti but know a days it is modified into metallic or plastic
syringe, this type of Anuvasana basti yantra maintaining the same pressure and
get good result. After the administration of Anuvasana dravya buttock are
patted by the hands three to four times. Heels and legs should be raised from
the cot for three times and massaged on the abdomen in anti-clock wise
direction by these methods sneha dravya will travel throughout the colon after
light diet is given.
Combination
of herbs in liquid form is administered through the rectal rought this
procedure is called niruha basti. For the administration of niruha basti use well
sterilized white bronzes with polished or disposable plastic basti netra, these
are very convenient to use. There is no any chemical changes in the basti dravya
may not cause much more adverse effect. Know a day’s infections like H I V and
other sexually transmitted disease causing organisms are most common moreover
these easily transmitted through the anal rought. So in this condition everyone
should use plastic disposable basti netra to avoid of spreeding of infections.
In
our practice there are to basti natra are used, one for male and another one
for female as our convennitent.
It
is very difficult to collect the animal bladder and sterilization method for
Basti putaka, There may be chance of spreading streptococcal infection, so use
of polythine bags or plastic covers is convents so in our practice two plastic
bags are used.
In
different area niruha basti practicing mainly two methods one is classical
method (basti netra, putaka), another one is Can method. Among those two
methods basti netra & putaka maintaining proper pressure and get very good
result.
According to our clinical experience the dose
of vagbhata i.e. 150 ml of makshika, 100 Gms of kalka, 15 Gms of saindhava
lavana, 150 ml of sneha and 500 ml of kashaya total 900 ml is the optimum dose
for niruha basti. In some cases they having fecal incontinence, usually common
in old age, multi system operations like heamoroidectomy etc. in that condition
decrease the dose up to 500 to 600 ml that is 100 ml of makshika, 100 ml of
sneha, 75 gms of kalka 10 gms saindhava lavana, 300 ml of kashaya it’s also
gives good results but take care about the evacuation of enema drava in middle.
For
the preparation of niruha basti dravya churning method is very convenient, and
for the administration of niruha basti classical method maintaining proper
pressure and get good result.
As per
the route word “vasu nivase” Means to reside or retained. The medicated oil
retained over the affected area or body part for a prescribed time such a
treatment technique is also called Basti. According to site of application name
has been given as Shiro basti, kati basti, Janu basti, Prusta basti, Uro Basti /
Hrudaya basti etc.
The
way of application of basti as using black gram power or basti caps and also
modification done in the poring of oil inside the wall i.e. with spoon, poring
with the help of coconut leaves or squesing with Catton.
Basti
practitioners are modified the Dose of the Basti dravya, Basti yanta (Basti
netra, Basti putaka and Whole Yantra).way of preparation of basti dravya, and
way of administration of basti dravya as its convence in different area but
ultimate goal is to achive the good therapeutic effect of the Basti Karma.
